- What stage of training or clinical practice are you currently in?
- What feels most unclear, misaligned, or challenging in your career right now?
- What draws you to lifestyle medicine, entrepreneurship, or nontraditional practice models?
- Are you currently building—or considering—a specific project, pivot, or venture? If so, please describe.
- What type of support or guidance are you hoping to receive through mentorship?
- What would a successful three months of mentorship look like for you?
- Are you able to commit to two mentorship sessions per month for a minimum of three months?
